JOB SUMMARY: The Lynn Community Health Center (LCHC) Clinical Pharmacist will provide medication management expertise in a Federally Qualified Health Center (FQHC) environment under a collaborative practice agreement. Through direct patient care, Embedded on a care team, the Clinical Pharmacist will leverage their clinical expertise and knowledge of medications to ensure our patients have better health outcomes.
